Revisions to Standard Form (SF) 1153, Claim for Unpaid Compensation of Deceased Civilian Employee

The Office of Personnel Management (OPM) announces a revision to Standard Form 1153, Claim for Unpaid Compensation of Deceased Civilian Employee. The function associated with Standard Form 1153 was assigned originally to the U.S. General Accounting Office, but was later realigned to OPM. The purpose of the revision is to provide users the ability to complete the form on the web and to clarify the language on the form. The revised form will supercede the previous form, published in April 1982. Previous editions should not be used. The form will be available on OPM's website at http://www.opm.gov/forms, and the General Services Administration (GSA) website at http://www.gsa.gov/forms. The form will no longer be stocked or available from the GSA, Federal Supply Service.
